Repair / Shipping Forms

Please thoroughly read the following before packaging unit:

• Carefully pack your unit for shipping (use the original box and packing material if possible).
• Please include the completed Service Form below with unit.
• Remove any mounting bracket(s) and mounting hardware from unit (National Pro Video is not responsible for any loss or damage to mounting brackets nor mounting hardware).
• Include the lamp and the factory lens assembly (no after-market lens please).
• Do not include any accessories, cables, manuals or remotes (only include the remote if unit is experiencing remote problems).
